If Yes, you will need to attach a copy of each complaint form filed in court and any subsequent court pleadings on the file attachments screen before submitting your complaint form. Upload any additional supporting documents related to your complaint, including governing documents such as the declaration, bylaws, articles of incorporation, rules, or other association records. Providing complete documentation helps prevent delays in the review process.

Governing documents may be obtained from the association. Condominium declarations may also be available through the Clerk of Court or official records office in the county where the condominium is located.
